Great review, I agree with most of your points, and I love how charater driven yet story-plot following this ep was!
Per your : "She gave up far too quickly on the defib on the patient" I totally agree about the sentiment, however just on a technicality, he went into asystole, which means no heart beat at all, and you can't shock a heart in asystole (only if it's in VT or Ventricular Tachycardia). Rather she shouldn't have given up, the next step would have been to administer Adrenalin (or Epinephrine if your American) to restart the heart!
